Person-Centered Planning

The Focus Is on the Individual

With the goal of improving the lives of adults with intellectual and developmental disabilities, Providence Center’s person-centered planning approach focuses on the individual and tailors programs and services to meet his or her personal goals emphasizing productive, meaningful roles in the community.

The primary goal of person-centered planning is to have the participant direct their funding plans to achieve what they want for themselves and design the supporting factors needed to accomplish that goal.

Building a Stronger Community

Person-centered planning has gained greater support from providers and government agencies in recent years but has always been a cornerstone of Providence Center. It has improved the transition of youth into adult programs and the outcomes for those we serve. This key strategy has increased awareness and outreach on behalf of those with intellectual and developmental disabilities, building stronger community support and new partnerships.

By emphasizing person-centered planning, Providence Center has focused on programs that enable those who we serve to live and work in the world. Our supported employment program and community partners are key to achieving each individual’s goals as well as a good quality of life. Community integration is critical to individuals with intellectual and  and developmental disabilities. Being involved and working in the community is key to gaining a fulfilling life with new experiences and positive relationships!

Supported Employment Program

Providence Center knows that every person should have their voice heard in how they want to live their lives and what they want for their future. Through open communication with families and caregivers, each person we serve receives the support they need to define and achieve their personal goals. Community involvement further enhances experiences and leads to greater independence.
